About the Role
The REACH Project is seeking a community-facing outreach staff member to provide housing and healthcare support to chronically unhoused populations. This grant-funded role focuses on facilitating access to leasing services, wraparound care, and social services while promoting health equity.
Responsibilities
- Provide on-site housing case management and outreach support
- Facilitate referrals for case management and arrear payments
- Manage annual renewal of Department of Social Services benefits and Section 8 vouchers
- Provide culturally appropriate health education, assessment, and advocacy
- Establish and maintain positive relationships with community individuals and housing authority staff
- Collaborate with a multidisciplinary team to address social determinants of health
- Obtain CRPA certification within the first six months of employment
- Facilitate patient access to support systems and transportation assistance
- Develop and manage support groups
- Conduct behavioral health screenings under supervision
- Participate in multidisciplinary team meetings and patient care huddles
- Maintain accurate and timely outreach activity notes
Requirements
- High school diploma or equivalent
- Minimum of two years of experience in human services preferred
- Experience navigating local medical and social support systems
- Lived experience preferred
- Commitment to harm reduction, health equity, diversity, and inclusion
