About the Role

The REACH Project is seeking a community-facing outreach staff member to provide housing and healthcare support to chronically unhoused populations. This grant-funded role focuses on facilitating access to leasing services, wraparound care, and social services while promoting health equity.

Responsibilities

  • Provide on-site housing case management and outreach support
  • Facilitate referrals for case management and arrear payments
  • Manage annual renewal of Department of Social Services benefits and Section 8 vouchers
  • Provide culturally appropriate health education, assessment, and advocacy
  • Establish and maintain positive relationships with community individuals and housing authority staff
  • Collaborate with a multidisciplinary team to address social determinants of health
  • Obtain CRPA certification within the first six months of employment
  • Facilitate patient access to support systems and transportation assistance
  • Develop and manage support groups
  • Conduct behavioral health screenings under supervision
  • Participate in multidisciplinary team meetings and patient care huddles
  • Maintain accurate and timely outreach activity notes

Requirements

  • High school diploma or equivalent
  • Minimum of two years of experience in human services preferred
  • Experience navigating local medical and social support systems
  • Lived experience preferred
  • Commitment to harm reduction, health equity, diversity, and inclusion
