Run monthly workshops covering key life skill competencies for youth as per ACS requirements.
Ensure connections to community resources.
Assist youths with completing housing applications (NYCHA, Emergency Housing Vouchers, etc.).
Obtain all vital documents for youths on caseload before discharge.
Provide supportive care for youths who leave placement before age 21, as per ACS regulations.
Document all contact in Connections within the required timeframe.
Complete PYA checklist using ACS web-based PAT tool.
Attend team house meetings and conferences as necessary.
Obtain and place youths in paid and unpaid internships based on skill level, assisting with placement through SYEP annually.
Collaborate with caseworkers and team members to ensure youths set and achieve goals.
Maintain monthly tracking grids and submit them timely to the Support Specialist and Program Director.
Reconcile monthly expenses in a timely manner.
Travel to various locations in Queens and Brooklyn where youths are housed.
Perform other duties as assigned.

Minimum of two years’ experience working with young adults.
Familiarity with the community to which assigned.
Knowledgeable about the major culture groups within the neighborhoods served.
Ability to set limits and intervene appropriately.
Be a team player.
Have strong sensitivity to cultural differences present among staff and clients.
Possess a strong belief in people’s ability to grow and change.
Forge a mutually respectful partnership with persons served and their families.

Working with developmentally disabled youth is a plus.
Knowledge of Motivational Interviewing is a plus.
Residence in the community where the position is located is desirable.
Being from the culture and ability to speak the language of the major groups served in the community is desirable.
Previous work or connection with the developmentally disabled population is desirable.
Ability to travel is required.

Interacts with community service providers to obtain and monitor delivery of services.
Interfaces with agency business support, maintenance services and quality improvement staff as well as other PYA staff.
