About the Role
The Treatment Coordinator serves as the primary point of contact for patients and families, facilitating admissions, transitions, and administrative support to ensure a positive patient experience.
Responsibilities
- Send welcome information to patients and families
- Identify referrals and coordinate with treatment teams to secure services
- Coordinate patient discharges and transfers to other facilities
- Manage patient admissions in medical records systems
- Register patients in the portal and prepare admission documentation
- Greet and assist facility visitors
- Manage confidentiality statements and vendor access
- Answer and direct incoming phone calls
- Maintain patient binders and discharge packets
- Schedule meeting spaces and manage facility common areas
- Oversee incoming and outgoing mail and packages
- Coordinate patient transportation
- Submit daily attendance and census reports
- Coordinate outside medical appointments for patients
Requirements
- High school diploma or GED
- Ability to work Monday through Friday, 7:30am-4:00pm
