About the Role
We are looking for nurses to join our team and provide palliative nursing care and symptom control. This role involves performing patient assessments, developing care plans, and documenting interventions in the EMR.
Responsibilities
- Perform initial and ongoing patient/family, physical, emotional, psycho-social, cultural and spiritual assessments.
- Coordinate the delivery of comprehensive, holistic services by the Interdisciplinary Hospice Team.
- Provide palliative nursing care and symptom control in accordance with physician orders and IDG plan of care.
- Document and enter into EMR all assessments and interventions before the end of the shift.
- Develop and prioritize the interdisciplinary care plan goals of the patient/family, hospice team and attending physician.
- Provide anticipatory guidance to the patient/family.
- Effectively communicate in English, both verbally and in writing.
- Revise nursing diagnosis, goals and interventions, expected outcomes, and plan of care based on assessment data and input from patients and family members.
Requirements
- Graduate of an accredited nursing school.
- Valid New York State nursing license.
- Minimum of at least 2 years of nursing experience required.
- Minimum of at least 1 year ICU, Med-Surg, ER or similar required.
- Ability to exercise critical thinking skills.
- Skill to prioritize and handle multiple tasks effectively.
- Ability to lift at least 30 pounds of supplies and/or equipment.
- Moderate/High level of possible exposure to infectious agents.
- Routine duties involve potential for direct exposure to blood or body fluids.
Skills
- Venipuncture and IV placement certification preferred
- Bilingual is a plus!
- English communication (verbal and written)
