About the Role

The Case Manager will conduct intake interviews, care plan development, care coordination, referral services, substance use counseling coordination, health education, and social services navigation to members with substance use and other chronic health conditions. This role will work with Ryan White and Harm Reduction prevention services for high-risk of HIV populations.

Responsibilities

• Completes initial assessment and reassessments on all assigned clients to develop individualized treatment goals.
• Establishes treatment goals and objectives with clients as part of the interdisciplinary team.
• Provides case management services including progress summaries, assessments, care planning, counseling, outreach, case conferences, advocacy, referrals, and discharge planning.
• Facilitates harm reduction individual and group services.
• Attends weekly/bi-weekly supervision meetings.
• Ensures timely submission of clinical documentation for uninterrupted services.
• Coordinates transitional care from inpatient service to other settings.
• Coordinates detox referrals and transportation services.
• Conducts prescreening and intake assessment interventions.
• Educates participants on Harm Reduction intervention and services.
• Conducts overdose prevention education and Narcan dispensation.
• Works collaboratively with team members to provide outreach and engagement to hard-to-reach clients.
• Maintains documentation and updates care plans in the electronic medical records system.
• Assists clients in navigating the health insurance system, including ADAP and other insurance networks.
• Coordinates with supervisor for escorts to life-saving services and home visits.
• Conducts field work and visits other provider settings to fulfill care plan goals.
• Meets assigned deadlines for progress note documentation and service coding.
• Participates in case conferences with medical providers and other stakeholders.
• Aids clients and families through advocacy and psychoeducation for entitlements and community services.
• Demonstrates understanding and sensitivity to chronic medical conditions.

• Participates in quality assurance and quality improvement planning.
• Participates in monthly staff development meetings and interventions.

Requirements

• Bachelor's degree in a human services field with at least two years of case management or social services experience, OR a High School diploma with at least 5 years of case management and/or care coordination services experience.
• Ability to make data entries into programs databases such as AIRS, e-Share, EiCare.
• Good group facilitation skills and overdose prevention training experience.
• Skilled with health insurance navigation and documentation.
• Good verbal, written, communication, and interpersonal skills.
• Proficient with Microsoft Office Suite (Word, Outlook, Excel).
• Culturally aware and sensitive to PWUD, PLWHA, LGBTQAI, YIDU, SMI, and MSM.
• Minimum of 2 years of case management experience with marginalized and high-risk populations.
• Experience working with adults with co-occurring substance use and non-chronic mental health disorders.
• Knowledge of community health and social service agencies, public benefits, and community resources.
• Training in Risk Reduction Counseling, HIV 101, Viral Hepatitis 101, Motivational Interviewing, LGBT Sensitivity, Syringe Exchange and Harm Reduction Training, HIV confidentiality and risk-reduction counseling, and Overdose Prevention Strategies is preferred.
• Bilingual in Spanish/English is preferred.
• Good understanding of Harm Reduction and its application in case management.
• Ability to remain stationary, stand, or sit for prolonged periods.
• Ability to move around to other worksites.
• Ability to occasionally lift and move objects up to 25 pounds.
