About the Role
The Care Manager will be an integral part of an initiative providing court and community-based, person-centered, voluntary support for individuals with unmet needs. This role involves engaging participants in screenings and assessments, identifying health needs, making referrals, and following up on service engagement. This is a growth position requiring collaboration with clinical professionals and court stakeholders.
Responsibilities
- Serve as a liaison between participants and CHN clinicians, collaborating with peers and MMU staff for seamless care transitions.
- Check insurance status and connect individuals to the NY State of Health Marketplace.
- Obtain baseline demographic information and provide real-time assessments of needs in coordination with clinical staff.
- Complete preliminary mental health assessments on-site and coordinate with supervisors/clinical teams to determine appropriate care levels.
- Complete detailed needs assessments to identify needs for supportive services (e.g., medication assistance, rehabilitative services, food pantry, housing) and ensure referrals are made.
- Ensure all relevant consent forms are completed and follow-up appointments are provided.
- Deliver services within best practices and document information accurately in computer systems.
- Identify potential barriers to care and strategies for overcoming them.
- Attend MMU meetings and associated trainings.
- Conduct client assessments and psychosocial interviews.
- Collaborate on individualized treatment plans.
- Provide ongoing individual and group substance abuse counseling services to a caseload.
- Maintain detailed case records.
- Engage in ongoing professional development.
- May require occasional travel to other NYC sites for program oversight, partner meetings, and trainings.
- Perform additional responsibilities as assigned.
Requirements
- Minimum of 1 year of experience in criminal justice, community mental health, or related health services preferred.
- Ability to demonstrate strong written and verbal communication skills.
- Ability to work independently with off-site supervision.
- Proficiency in MS Office.
- Working knowledge of MAPP, ePaces, PSYCKES is a plus.
- Bilingual in Spanish is a plus.
- Ability to perform additional responsibilities as needed.
- Relevant personal experience is a plus.
