About the Role

The Nurse Practitioner will collaborate with physicians and healthcare professionals to deliver high-quality, patient-centered care. This role involves diagnosing and managing acute and chronic conditions, promoting preventive care, and building strong patient relationships.

Responsibilities

  • Assess and interpret medical and psycho-social histories; perform physical examinations, identify problems, formulate treatment plans, provide education and referral, and evaluate intervention effectiveness.
  • Consult with or refer clients with abnormal conditions or other medical, psychological, or social problems to a collaborating physician or appropriate referral source.
  • Perform or order laboratory tests, interpret results, and report communicable diseases to the appropriate Health Department.
  • Order medications.
  • Follow established treatment guidelines as indicated by Health Center protocols.
  • Conduct health teaching and counseling during medical encounters to enable informed client choices about health and treatment options.
  • Complete client clinical record documentation in a timely manner according to health center policies and procedures.
  • Understand medical emergency procedures and be familiar with the location of emergency kits/supplies.
  • Participate in quality improvement/assurance activities.
  • Participate in continuing medical education activities to enhance professionalism and ensure quality of care.
  • Provide on-call coverage as scheduled.
  • Collaborate with all members of the health center to build a supportive team.
  • Apply infection control measures according to Health Center Policies and Procedures.
  • Maintain professionalism and adhere to client confidentiality.

Requirements

  • Licensed to practice as a NYS Nurse Practitioner.
  • Licensed and currently registered as a Registered Nurse in the State of New York.
  • 2 to 4 years of experience as a Nurse Practitioner.
  • Experience and knowledge of HIV/AIDS primary care preferred.
  • Experience and knowledge of managing clients with substance abuse preferred.
  • Previous experience with an electronic health record preferred.
  • Visual acuity and manual dexterity to interface with the computer and perform medical procedures.
  • Auditory acuity to handle phone calls and interact extensively with patients.
  • Maintains CMEs/CEUs required for licensure.
  • Maintains CPR certifications.
  • Maintains Infection Control certification.
  • Maintains DEA registration.
  • Certified by ANCC or AANP.
  • Bilingual English/Spanish preferred, but not required.
  • Preference for individuals with experience working in a community-based setting, particularly with patients who are LGTBQIA+, and/or have experienced homelessness, substance use, and/or mental health challenges.
  • Employees working in Health Services division must be fully vaccinated against COVID-19, unless approved for a legal accommodation.

Experience Level

  • 2 to 4 years of experience as a Nurse Practitioner

Salary/Compensations

  • $140,000-$150,000 annually
