About the Role
Community Access is seeking dedicated professionals to join our Assertive Community Treatment (ACT) and Intensive Mobile Team (IMT) programs. These roles are crucial in providing evidence-based, mobile treatment, rehabilitation, and support services to individuals living with serious mental health conditions, particularly those experiencing homelessness in NYC. You will work collaboratively within multi-disciplinary teams to help participants achieve their personal goals in housing, health, social, work, and education.
Responsibilities
- Provide holistic recovery services tailored to individual needs.
- Offer ongoing supportive counseling and service coordination.
- Integrate person-centered goals and services with team tasks.
- Provide psycho-education individually and in groups.
- Assist participants in honing vocational goals and finding relevant programs.
- Engage participants through individualized assessment and therapeutic intervention planning.
- Utilize evidence-based practices and harm reduction strategies.
- Serve as a clinical resource to team members on mental, behavioral health, and co-occurring substance use issues.
- Assess and monitor participants’ clinical health and response to treatment.
- Assist individuals in achieving mental health, medical, rehabilitation, and recovery goals.
- Complete assigned assessments and service plans on time.
- Complete progress notes for all contacts, collaterals, and attempts within two business days.
- Update face sheets for new diagnostic and prescription information within 2 business days of any hospital contact.
- Complete assigned nursing evaluations on time.
Requirements
- Have an understanding, appreciation, and commitment to the philosophy and mission of Community Access.
- Commitment to person-centered treatment strategies, upholding participants’ rights, and self-determination in service provision.
- Knowledge of New York City and/or New York State vocational programs and services.
- Knowledge of entitlements systems.
- Ability to prioritize and meet deadlines.
- Be creative and flexible.
- Able to show initiative and be responsible for follow-through.
- Ability to work as part of a multi-disciplinary team.
- Skilled in conflict mediation/negotiation and have an assertive approach to problem solving.
- Must be fingerprinted and cleared by the New York State Justice Center.
- Ability to utilize public transportation throughout the city for assigned tasks.
- Available to work a flexible schedule, mornings, evenings and weekends in response to participant needs.
- Must be able to work in the community, including use of public transportation.
- Must be able to lift and carry 25 pounds.
Skills
- Excellent oral, writing, and listening skills.
- Computer proficiency, Microsoft Word, Excel.
- Comfort with learning new systems and paperless record keeping.
- Computer proficiency in Windows operating systems and programs, such as MS Word, Excel, PowerPoint.
- Comfort with learning new electronic systems.
- Ability to synthesize and summarize information and make judgments regarding care.
- Bilingual Spanish-speaking strongly preferred.
- Bilingual Spanish-speaking preferred.

Experience Level
- Minimum of two (2) years’ experience working with individuals with a mental health condition with co-occurring substance use disorder.
- Minimum two (2) years’ experience working as an LPN.
- Minimum two (2) years’ experience working with persons with mental health conditions in health or social services setting.
Education Level
- Licensed Master of Social Work, Licensed Mental Health Counselor or Masters degree in similar field.
- Minimum of Master’s Degree in Social Work, Psychology, Mental Health Counseling, or other Health and Human Services related field; LMSW preferred.
- Currently licensed as a Licensed Practical Nurse by the New York State Education Department.
