About the Role
Heights and Hills, an affiliate of Greenwich House, is seeking a Case Management Supervisor to oversee daily operations, support case managers, and ensure program goals are met. This role focuses on promoting successful aging for homebound older adults in Brooklyn through professional, community-based services.
Responsibilities
- Oversee the Department for the Aging-funded Case Management program
- Supervise five case managers in their daily work
- Collaborate with the Program Director and Intake Coordinator to assign cases
- Review case records for DFTA standards compliance and clinical appropriateness
- Identify staff needs and develop in-service training programs
- Supervise and coordinate activities for student interns
- Represent the agency at community meetings
- Conduct outreach to community leaders and organizations to identify service needs and gaps
- Forge community partnerships to enhance the lives of seniors
- Work on special projects with the Case Management Director
- Compile statistics, prepare reports, and maintain attendance and required records
Requirements
- LMSW or LCSW certification
- Two years of experience working with the elderly
- Understanding of biological, psychological, and social aspects of aging
- Knowledge of community resources and public benefits eligibility
- SIFI Certified or eligible (3 years post-Masters)
- Ability to travel via public transportation for community presentations and home visits
- Bilingual skills in Spanish, Chinese, or Haitian Creole preferred
- Knowledge of Department for the Aging case management standards preferred
- Familiarity with the Vive database preferred
