About the Role
The RN Case Manager will administer skilled nursing care to patients requiring professional nursing service, teach and supervise the family and other members of the nursing team, and perform these services in accordance with the physician’s orders.
Responsibilities
- Provide and coordinate quality patient care, maintain a full caseload, and meet productivity goals with accurate, timely documentation.
- Manage each patient’s care efficiently and cost-effectively to achieve positive patient outcomes.
- Oversee the overall plan of care and its outcomes, including care delivered by other nursing staff.
- Conduct patient admissions by obtaining medical history, performing physical examinations, and evaluating the patient's environment and family support.
- Determine the need for home health aide services and their frequency.
- Interpret agency services to patients and families as part of care planning.
- Meet regulatory requirements and understand various insurance guidelines.
- Practice accepted nursing standards for infection control.
- Develop and implement the nursing care plan.
- Provide skilled nursing care, treatments, and preventive procedures as ordered by the physician.
- Initiate preventive and rehabilitative nursing procedures.
- Observe signs and symptoms, report reactions to treatments, and note changes in patient condition to the physician.
- Teach, supervise, and counsel patients and caregivers regarding nursing care needs.
- Oversee and evaluate care given by Home Health Aides.
- Utilize computer skills for accurate documentation.
- Document services rendered, changes in patient condition or home situation, revise care plans, record supervisory visits, evaluate patient care, and close charts of discharged patients.
- Perform other duties as assigned.
Requirements
- Current, valid RN license issued by NYS Department of Education required.
- Minimum one year home care experience.
- Knowledge of State and Federal regulations and payer guidelines.
- Ability to exert up to 50 pounds of force occasionally for medium work.
