Responsibilities
- Administer medications and treatments as prescribed by the physician.
- Monitor and record vital signs, such as blood pressure, pulse, and respiration rate.
- Assist with activities of daily living, including bathing, dressing, and grooming.
- Provide wound care, including cleaning and dressing wounds.
- Monitor and document changes in residents' health status and report any concerns to the AL/PC Administrator.
- Provide in-dwelling catheter care occasionally when needed.
- Provide wound-care occasionally when needed.
- Assist with meal preparation and feeding, as needed.
- Assist with mobility, transfers, and positioning of residents.
- Document all care provided and maintain accurate resident records.
- Communicate effectively with residents, families, and other healthcare professionals.
- Adhere to all policies and procedures related to resident care and safety.
- Assist in creating, updating and maintain resident care plans as necessary.
- Participate in continuing education and training to maintain licensure and stay current with nursing practices through Relias on-line training as required under PA Regulations on an annual basis.
Requirements
- HS Diploma or equivalent (ie: GED).
- Successful completion of an accredited Licensed Practical Nurse training program.
- Current Pennsylvania LPN License with no judgments or disciplinary action.
- CPR Certified
- First Aid Certified
- AED Certified (Preferred)
- 1 year experience working as an LPN in long term care.
- Experience in a memory care facility is preferred but not required.
- Must have knowledge of nursing practices and procedures.
- Must be able to communicate effectively with residents/patients, families, and healthcare providers.
- Must be able to work as a team member and follow directions.
- Must be able to handle stressful situations and respond appropriately.
- Mandatory Influenza Vaccination Employer (October-March) unless an exemption is approved.
- COVID-19 Vaccinations are suggested but not required at this time.
