About the Role
Coordinates patient services under the direction of the team manager/designee, including prioritization and coordination of daily patient schedules, requesting additional services, and ensuring timely assignment of referrals.
Responsibilities
- Manages multiple phone calls in a timely and professional manner providing appropriate follow up.
- Provides field staff assistance and resources for day to day issues.
- Checks email in a timely manner providing follow up.
- Collects, organizes and reviews various programs for information and reporting and initiates follow up.
- Updates the EMR per agency standards.
- Performs functions related to transfers or discharges as required by regulation.
- Builds and maintains strong, communicative relationships with the team.
- Provides exemplary customer service.
- Performs general office duties/projects including answering phones, documentation/order processing, copying, filing, mailing, and obtaining/ordering office supplies.
- Assists in orientation of new staff, role modeling, participates in quality assurance activities.
- Attends team meetings and other internal meetings as requested.
- Other duties as assigned.
- Responsible for coordinating participant services and staff scheduling for ElderONE.
- Assures complete and accurate schedules for ElderONE staff.
- Creates and schedules case assignments for ElderONE.
- Assists with timekeeping for ElderONE as needed.
- Provides a range of supportive services to Home Health Aide operations of ElderONE.
- Assures appropriate routing of calls and assistance with transitioning participants through care for ElderONE.
Requirements
- Excellent interpersonal skills and customer service skills.
- Ability to multi-task in a fast paced environment.
- Proficient use of Microsoft office.
- One year of experience in home care setting or medical office preferred.
- For ElderONE only: 1 year of experience working with a frail or elderly population preferred.
