About the Role

FOX Rehabilitation is seeking passionate, licensed Physical Therapists to deliver one-on-one care to older adults in their homes or senior living communities. Using the Geriatric House Calls™ model, you will help patients improve mobility and independence while enjoying autonomy over your schedule and the ability to incorporate telehealth.

Responsibilities

• Provide one-on-one physical therapy in private residences and senior living communities
• Perform evaluations and develop individualized plans of care
• Deliver evidence-based treatment to support patient wellness and functional independence
• Document patient care effectively within the EMR system
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff on safety and aging in place
• Provide telehealth and virtual care services

Requirements

• Valid Physical Therapy license in the state of practice or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification
• Ability to lift 25–50 lbs occasionally
• Ability to perform CPR
• Ability to provide moderate to maximum assistance with patient transfers and bed mobility
• Ability to reposition patients, stand/walk for extended periods, and climb stairs
• Ability to drive to patient homes and senior living communities
• Ability to wear PPE when required
