About the Role
Provide direct case management services including assessment, counseling, crisis intervention, referral, and advocacy to ensure an effective social service support plan is in place to enable the successful relocation of families to permanent housing.
Responsibilities
- Utilize the Department of Homeless Services (DHS) - Client Assistance and Re-housing Enterprise System (CARES) to complete family intake assessments, service plans, housing plans, and incident reports.
- Review accessible information from Human Resource Administration, Administration for Children Services, Department of Probation, and Department of Homeless Services using CARES.
- Ensure all information in the CARES system is updated in a timely fashion as required by DHS.
- Provide advocacy and referral services to ensure clients receive appropriate benefits, entitlements, and services to enhance their transition to permanent housing.
- Coordinate services to stabilize the family unit, address substance abuse issues, and improve life skills to achieve permanent housing.
- Assess client needs, formulate service plans, monitor progress, and document all significant interactions.
- Develop and maintain helping relationships with families to assist them in achieving service goals.
- Provide individual counseling and crisis intervention services to help stabilize families and minimize the impact of displacement.
- Maintain familiarity with program resources and refer families to appropriate off-site and on-site resources.
- Facilitate groups to address family issues, enhance life skills, and assist families in achieving self-sufficiency.
- Coordinate client services provided by other social services staff to ensure timely and appropriate service delivery.
- Facilitate effective communication among staff, ensuring timely and accurate transfer of relevant information to administrative staff.
- Perform other duties and special projects as assigned, including serving on-call.
- Assist in transferring clients in and out of the program.
- Facilitate communication with other staff in an interdisciplinary context.
- Participate in staff meetings and contribute to policy and procedure development.
- Participate in training and staff-development activities to acquire and maintain necessary skills.
Requirements
- Experience facilitating groups for homeless adults.
- Specialized knowledge of substance abuse issues and treatment.
- Specialized knowledge of family dynamics treatment.
- Bilingual language ability highly desirable.
- Excellent written and verbal communication skills.
- Excellent interpersonal skills.
- Computer literate in Microsoft Office.
- May be required to work long hours and a varied schedule.
- Must be able to travel throughout various locations in the New York City area.
- Ability to perform tasks in environments with climatic issues of temperature variance and ventilation.
- Ability to lift and move objects weighing up to 30 pounds several times a week.
