About the Role

The Home Based Crisis Intervention (HBCI) team provides intensive crisis intervention to children and families in Jefferson County. The program serves youth ages 5 to 20 at risk of psychiatric hospitalization or out-of-home placement, focusing on stabilization and connecting families with long-term support services.

Responsibilities

  • Respond to referrals within 48 hours
  • Meet with youth and families to discuss program options and service provision
  • Complete admission paperwork and obtain necessary consents
  • Collaborate with existing care managers, therapists, and psychiatric providers
  • Provide intensive crisis intervention and supportive counseling
  • Participate in team meetings
  • Coordinate with community agencies for long-term planning
  • Refer families to mental health services and other programs
  • Maintain thorough documentation of all case-related activities
  • Represent the organization as a liaison with service providers
  • Complete mandated reports and forms in a timely manner
  • Promote the program and provide community education
  • Participate in an after-hours on-call rotation

Requirements

  • Bachelor’s Degree in a Human Services field
  • Minimum of two years of relevant experience in community-based mental health or case management
  • Valid NYS driver’s license
  • Ability to maintain privacy of confidential information
  • Successful completion of pre-employment requirements including background checks and fingerprinting
  • Ability to complete required training in HIPAA, workplace harassment, and crisis intervention
