About the Role

The Home Based Crisis Intervention (HBCI) team delivers intensive crisis intervention to children and families within Jefferson County, serving youth aged 5 to 20 at imminent risk of psychiatric hospitalization or group treatment admission. The HBCI Interventionist provides direct crisis intervention, stabilization, and case management services, linking families with necessary resources.

Responsibilities

• Respond to each referral within 48-hours.
• Meet with each youth and family/guardian to discuss program options, service provision, and alternative service options.
• Conduct all admission paperwork and ensure all necessary consents/releases are signed.
• Collaborate with already established providers such as care managers, therapists, and psychiatric providers.
• Provide intensive crisis intervention and supportive counseling to the youth and family.
• Participate in team meetings.
• Work with community agencies, organizations, and systems for long-term planning and support of the youth and families served.
• Make referrals to mental health services and other programs as needed.
• Provide thorough documentation regarding all case related activities including face to face contacts, phone calls, case conferences, clinical interventions, and noted incidents.
• Represent CHJC as a liaison with various private and public service providers and resource agencies.
• Complete all CHJC and New York State mandated reports and forms in a timely fashion.
• Continuously promote the program and provide community education.
• Participate in an after-hours on-call rotation.
• Perform other duties as assigned or requested.

Requirements

• Must maintain privacy of confidential information.
• Successful completion of routine pre-employment requirements as mandated by the New York State Department of Health, Office of Mental Health and Office of Children and Family Services along with ongoing clearances and exclusion checks.
• Requirements include employment health requirements, reference checks, criminal background checks/fingerprinting, State Central Registry clearance and eligibility to work in the United States.
• A valid NYS driver’s license is required.
• Corporate Compliance, HIPAA Privacy Security, and Workplace Harassment and Discrimination Training and Nonviolent Crisis Intervention, First Aid and CPR will be required following date of hire.
• May have contact with service recipients that are agitated and/or are in crisis.
• Some risk involved working with service recipients with mental illness.
• Must be able to sit, stand, walk, lift, carry, push/pull, climb, bend, and stoop.
• Must be able to perform fine motor skills, read, and type.
• Must be able to sit for long periods of time.

Experience Level
