About the Role

The Program Manager serves as an organizational lead for the Permanent Supportive Housing Health Care Collaborative (PSH HCC), responsible for the development, implementation, and strategic direction of clinical care services. This role leverages a clinical background to guide staff, improve quality, and ensure integrated, wraparound medical and behavioral health care for individuals transitioning out of homelessness in Travis County.

Responsibilities

Analyze population health and program performance data to improve clinical workflows and patient outcomes.
Lead the development of key performance metrics, program deliverables, and outcome reporting.
Support the implementation, oversight, and continuous improvement of the PSH program.
Coordinate program activities and resources with internal teams and external partners.
Prepare program reports and data summaries for leadership and stakeholders.
Monitor budgets and track expenditures to ensure alignment with funding guidelines.
Apply trauma-informed, culturally responsive, and harm reduction principles to program design.
Represent the program in organizational and community settings.
Provide programmatic guidance, onboarding, training, and coaching to staff.
Develop and implement patient-centered workflows and best practices.
Build and maintain collaborative relationships with internal and external stakeholders.
Identify program risks and performance gaps; recommend and implement solutions.
Collaborate with the care management team to support continuity of care.
Lead care team huddles and staff meetings to ensure clinical alignment.
Monitor and evaluate patient and clinical team experiences.
Ensure program compliance with regulatory, accreditation, and organizational standards.
Engage with patients in a non-clinical capacity to foster trust.
Maintain required licensure and professional development.
Oversee quality assurance activities, chart audits, and adherence to care protocols.
Ensure staff are trained in emergency response and behavioral health crisis protocols.
Serve as operational site lead in the absence of designated leadership.

Requirements

Bachelor's degree in Nursing, Social Work, Healthcare Administration, Public Health, or a related field.
3 years of experience managing clinical projects, clinical operations, or working in a medical office in a managerial role.
1 year of experience in community-based settings.
2 years of experience working with marginalized populations, including individuals experiencing homelessness, justice involvement, severe mental illness, and substance use disorders.
2 years of experience and proficiency with healthcare tools, technology, and systems (e.g., Microsoft Office, EMR systems).
Basic Life Support (BLS) certification for Healthcare Providers from the American Heart Association upon hire.
