About the Role
Responsible for overseeing all case work services, ensuring the completion of all required paperwork and full compliance with City and State RHY regulations and standards.
Responsibilities
- Complete Initial Service Plan Review & Case Conferences for clients/families & facilitate additional family meetings and/or program visits.
- Coordinate services identified in the Initial Service Plan & subsequent reviews to assist clients in meeting their short & long-term goals.
- Complete home assessments for clients who will be utilizing weekend passes.
- Report any suspected abuse or neglect to Justice Center/State Central Register.
- Become DYCD Connect trained & proficient.
- Oversee Counseling Specialists regarding their specific responsibilities supporting case management services.
- Provide aftercare services to clients for 90-days post discharge.
- Maintain case records & ensure necessary documents are present on all current residents in their 90-day aftercare period.
- Facilitate weekly force field/team meeting.
- Conducts initial intake interviews of prospective residents in the absence of Intake Coordinator.
- Work collaboratively with Program Coordinator, Counseling Specialists and Intake Coordinator to ensure quality of service delivery.
- Attend & Participate in QI/QA & program admin meetings.
- Participate in staff and/or team meetings.
- Perform other duties as assigned.
- Responsible to maintain a caseload.
