About the Role
The Special Teams Home Health Aide position requires availability to support the on-call, congregate, Private Pay, or CHHA teams, with a minimum of 16 hours per month on weekends. This role involves accepting all assignments during designated shifts and requires a high level of professionalism, compassion, and empathy.
Responsibilities
- Be available during designated shifts and accept all assignments.
- Work with cases involving cats/dogs and smoking, and be willing to work with all types of clients.
- Adhere to arrival/departure times, arranging deviations with supervisors.
- Receive specialized training in topics such as NHTDW, Companion Care, Memory Care, and Palliative Care.
- Exercise professionalism, confidentiality, compassion, and empathy.
- Be available for on-call duties, including 24-hour cases.
- Provide health-related services as outlined in the plan of care.
- Assist with personal hygiene, housekeeping, and other supportive tasks in the client's home.
- Immediately report changes in client's condition to the supervising professional.
- Complete and submit necessary documentation, including charting tasks and observations on an android device.
- Attend ongoing in-service programs to maintain certification.
- Ensure the safety of the client, client's family, and employee.
- Maintain HIPAA compliance.
- Provide own transportation to and from assignments and arrive promptly.
- Commit to being available for work and accepting assignments.
- Speak and write in English to follow the plan of care and communicate effectively.
- Perform other duties as assigned.
Requirements
- HHA certification in good-standing, according to NYS regulations.
- Mature and able to deal effectively with the job and its demands.
- Respectful of client's rights and treat clients and their belongings with dignity and respect.
- Have a sympathetic attitude towards the care of the sick and disabled.
- Availability requirements are 40 hours per week for Full-time.
- Currently have and maintain an active/valid driver’s license.
- Must have reliable transportation.
- Must have a working phone and communicate DAILY with the ASC.
