About the Role

The Intake Coordinator plays a crucial role in receiving referrals, gathering patient information, and ensuring a smooth transition to home care services, all while providing superior customer service.
Responsibilities
- Manage multiple phone calls professionally from hospitals, physician offices, patients, and family caregivers.
- Handle incoming inquiries and referrals to the Unity CHHA.
- Coordinate with home care coordinators and the Home and Community Based Services team for seamless transitions.
- Obtain all necessary patient information, including demographics, reason for referral, medical diagnosis, and social information.
- Collaborate with referral sources to ensure completeness and accuracy of medical information.
- Process new referrals by registering patients in the computer system and preparing charts.
- Document data in the electronic medical record referral area.
- Obtain required insurance information and authorizations.
- Initiate the process to obtain necessary "Face to Face MD" forms.
- Review new referral specifics with the manager.
- Collect data and complete assigned reports.
- Participate in relevant committees and groups.
Requirements
- Graduate of an accredited school of practical nursing.
- 1-2 years of experience in an office setting answering multiple phone lines.
- 4 years of clinical experience preferred.
- One year in the home care setting preferred.
- Experience with an electronic medical record.
- Proficient use of Microsoft Office.
- Working knowledge of Home care admission criteria/regulations.
Skills
- Excellent interpersonal skills.
- Ability to multi-task in a fast-paced environment.
- Registered in NYS as a licensed practical nurse professional.
