About the Role

As a Crisis Coordinator, you will develop, maintain, and facilitate outreach and client engagement activities to support advocacy and linkage to BHSN and community services.

Responsibilities

  • Assist crisis team to perform outreach to clients, established and referred to support engagement and bridge the time between scheduling/discharge and appointment date.
  • Supports crisis team with available resources both internal and external to ensure engagement and remove barriers, triaging incoming calls, follow up calls, and on call support.
  • Alters processes as necessary for optimized efficiency and results of providing crisis intervention, tracking, and reporting.
  • Links clients to appropriate community resources such as mental health providers, primary care physicians, and supportive services, including coordinating services between internal BHSN programs including but not limited to; PROS and Adult Clinic.
  • Coordinates outreach activities with MHU discharges, with client contact and serving as liaison to MHU – CVPH and other higher levels of care.
  • Initiates interventions to address challenges related to the social determinants of health.
  • Participates in various meetings, both internal and with referral sources.
  • Provides follow up and outreach to missed client appointments, intakes, identifies barriers to services and coordinates with supportive resources.
  • Documents services in their entirety within a timely manner in accordance with regulatory bodies and organizational policies/procedures including collecting insurance, demographics, social determinants of health, etc.
  • Maintains client confidentiality and confidential health information.
  • Addresses quality outcome measures by utilizing the agency’s electronic health record and electronic databases, such as, but not limited to PSYCKES, and improves team actions as necessary to improve quality outcome measures.
  • Support Crisis Director / Manager with admin tasks to ensure productivity and effective utilization of crisis services.
  • Other duties as assigned.

Requirements

  • Ability to successfully lead a diverse team and improve client health outcomes.
  • Strong engagement skills as well as written and verbal communication abilities required.
  • Easily builds strong rapport with others, based on trust, compassion, and awareness of professional boundaries.
  • Valid NYS driver’s license with driving record acceptable to BHSN insurance carrier and reliable transportation for local travel required.
  • Ability to be on call as required.
