About the Role

Provide crisis risk assessments, stabilization, interventions, de-escalation, and follow-up services to clients experiencing emotional, behavioral, and psychiatric crises. Interventions are delivered via telephone and on-site mobile visits using a strength-based, trauma-informed, systems-oriented perspective. This includes developing crisis prevention and safety plans, coordinating services, and linking clients to community resources.

Responsibilities

  • Provide rapid response to adults, children, adolescents, and families in crisis via phone and on-site.
  • Participate in acute care emergency services rotation, including holidays and weekends.
  • Conduct risk assessments with diagnosis, mental status examinations, and substance abuse screening.
  • Analyze information to develop crisis stabilization, safety, and prevention plans, and follow-up plans.
  • Consult with team members and supervisors.
  • Coordinate with community resources for collateral input and follow-up care, including referrals.
  • Provide short-term crisis intervention and stabilization services in the home or community.

Requirements

  • Must have a valid NYS Drivers License.
  • Willing to travel to clients' homes and communities under various circumstances and weather conditions.
  • Willing to work in diverse settings, including clients' homes, potentially with pets, cigarette smoke, and other irritants.
  • Rotating weekend and holiday shifts required.
  • MSW -LP holders must be license eligible and willing to apply for a limited permit upon hire.
  • MHC- LP holders must already have obtained their limited permit and be close to finishing required clinical hours.
  • Willing to pass and obtain licensure within 3 months of hire.

Salary/Compensations

  • $29.00 - $36.00 Hourly
