About the Role

The licensed practical nurse performs tasks and responsibilities within the framework of casefinding, health teaching, health counseling, and supportive and restorative care under the direction of a licensed health care provider.

Responsibilities

• Collects patient data including vital signs, allergies, current medications, medical history, social history, family history, and chief complaint, documenting appropriately in the electronic medical record.
• Identifies practice problems and communicates them to management.
• Maintains a clean and organized department to ensure a safe patient care environment.
• Supports health care team initiatives and promotes excellence in customer service.
• Works with the PPM team to resolve clinical issues and identify improvement opportunities.
• Participates in ad hoc projects as assigned.
• Collaborates with the PPM team to address daily operational issues and solve problems to ensure quality patient care.
• Integrates CPH mission and core values within sites.
• Works in collaboration with CPH and PPM Departments as warranted.
• Performs and documents direct patient and family-centered care according to nursing standards of practice.
• Prepares charts for providers to ensure necessary information is present before patient appointments.
• Provides routine nursing care and treatment, collects specimens, performs medication reconciliation and administration, and assists providers with simple procedures.
• Supports and adheres to administrative and nursing service policies and procedures.
• Efficiently uses medical and clerical supplies to control practice expenses.
• Documents all treatments performed during patient encounters and adds the appropriate CPT code.
• Collaborates professionally with all members of the health care model.
• Works with the PPM team to comply with all established standards required under the accreditation standards of DNV.
• Is knowledgeable of state and federal health care standards.
• Adheres to CPH and PPM policies, procedures, guidelines, and protocols.

Requirements

• Graduate of a practical nursing program
• Current, unrestricted New York State practical nursing license
• BLS within three months of hire

Skills

• Knowledge of medications, their administration, and untoward effects

• 40 scheduled weekly hours

• 7:30 am-4:00 pm

Salary/Compensations

• $26.54 - $38.89
