About the Role

Community Habilitation Workers provide person-centered supports and services to individuals with developmental disabilities, focusing on personal choice, independence, and community integration. The role emphasizes guidance, training, and encouragement to help individuals live healthier and fuller lives, while also ensuring staff well-being.

Responsibilities

  • Provide services in the home and community according to the Habilitation Plan and individual Safeguards.
  • Document Habilitation services contemporaneously per Agency, OPWDD, Medicaid, and other requirements.
  • Contribute to the development and implementation of the Staff Action Plan based on the individual’s Life Plan.
  • Monitor the safety and welfare of individuals served, taking corrective actions and notifying supervisors of concerns.
  • Provide the required level of supervision as stated in the Staff Action Plan and individual Safeguards.
  • Assist individuals served in building positive community relationships.
  • Support individuals served in advocating for their needs and rights.
  • Provide training and encouragement for personal health care as needed.
  • Communicate health care concerns to supervisors or administrators.
  • Communicate professionally with individuals served, families, team members, and the community.
  • Complete all required paperwork and documentation daily, including progress notes, incident reports, and attendance records.
  • Complete Monthly Summary Notes on time.
  • Encourage community integration with an emphasis on inclusion, access, awareness, and safety.
  • Follow transportation protocol and conduct attendance checks when using an agency vehicle.
  • Attend staff meetings and participate in supervision for professional growth.
  • Complete all required initial, annual, and ongoing training.
  • Conduct fire drills and document results as instructed.
  • Report to work promptly and adhere to the assigned schedule.
  • Work overtime as needed to ensure proper staffing ratios.
  • Adhere to the NADSP Code of Ethics, Justice Center Code of Conduct, and NYS DSP Core Competencies.
  • Adhere to all Agency and Program Policies and Procedures, including HIPAA, Confidentiality, and the Corporate Compliance Plan.
  • Perform other related duties as assigned.

  • Ability to instruct others on vocational or daily living skills.
  • Ability to actively listen and communicate verbally and in writing.
  • Ability to work as part of an interdisciplinary team.
  • Good interpersonal skills and commitment to assisting individuals with disabilities.
  • Ability to maintain composure under pressure.
  • Ability to speak, read, and write in English.

  • Ability to use a computer or mobile device to access email and required software.
  • Ability to self-initiate projects and function autonomously when needed.
  • Good oral and written communication skills.
  • Ability to maintain confidentiality.

  • Good analytical ability with problem-solving and follow-up.
  • Ability to work independently and handle multiple tasks simultaneously.
  • Proficiency in the use of a personal computer and appropriate software and E-Mail system.
  • Ability to perform essential physical functions including stooping, lifting, kneeling, pushing, pulling, reaching, standing, walking, hearing, listening, and communicating appropriately.
