About the Role

To provide a safe and pleasant home-like atmosphere, emphasizing residents’ individuality while observing any changes in behavior and physical needs. Responsibilities include home management tasks and services, life enrichment experiences, personal care for residents, and delegated nursing services designed to maintain clients’ physical and emotional wellbeing.

Responsibilities

• Assist residents with personal care, including dressing, grooming, oral care, hair shampooing, bathing, lotion application, and nail/foot care as instructed.
• Assist residents with toileting, including commode, urinal, toilet use, and adult incontinence products.
• Assist with ambulation and transfers, utilizing proper body mechanics, transfer belts, and assistive devices.
• Assist residents in using hearing aids and glasses.
• Provide medication reminders, assistance, or administration as delegated by an RN.
• Prepare meals according to directed plans, including special meal requirements.
• Record temperature, blood pressure, pulse, and respiration, reporting any changes or concerns to the RN.
• Record blood glucose levels, reporting any changes or concerns to the RN.
• Assist with insulin injections as instructed, reporting any changes or concerns to the RN.
• Notify the RN of any resident or family complaints or concerns.
• Help residents achieve and maintain their highest level of performance and independence by promoting self-help skills and offering choices.
• Establish rapport with residents, lead activities, and provide socialization and companionship.
• Assist with treatments delegated by the RN or therapy tasks assigned by a licensed health professional.
• Observe residents and report changes in physical or emotional condition to the LPN or RN.
• Assist coworkers with daily responsibilities as needed.
• Organize and prioritize assigned work to complete tasks efficiently during the scheduled shift.
• Accurately document services provided, adhering to agency policies and completing all required paperwork legibly and in a timely manner.
• Attend required in-service training (at least 8 hours per 12 months), staff meetings, and other agency events.
• Follow agency procedures for universal precautions.
• Maintain a clean environment for clients, including dusting, vacuuming, mopping, and other household chores.
• Assist with life enrichment experiences that engage residents' minds, bodies, or spirits, and lead or coordinate activities as directed.
• Self-initiate engagement in life enrichment experiences.
• Provide individual life enrichment experiences as directed.
• Utilize validation techniques and appropriate diversion activities for clients with dementia.
• Adhere to the agency dress code, wear a name tag, and maintain good personal hygiene and grooming.
• Follow agency policies regarding safety rules and report any safety issues, concerns, or injuries immediately to the LPN or RN.

Requirements

• Graduate of a recognized nursing assistant program and in good standing with the Minnesota NA/registry, OR successful completion of the agency’s orientation and training program for unlicensed personnel and demonstrated competence by an RN.
• Previous health care or geriatric experience is desirable.

• Ability to work flexible times, including weekends, evenings, and holidays.
• Experience working with older adults; previous experience in a hospital, nursing home, or home care agency is preferred.
• Dependable and punctual with a good attendance record.
• Adherence to building dress code, good personal hygiene, and grooming.
• Ability to prioritize and organize work effectively and efficiently.
• Ability to work as a team member with other personnel, the LPN, and RN.
• Ability to follow directions accurately and perform assigned or delegated tasks.
• Willingness to learn and adapt to changing environments and job functions.
• Must receive a “not disqualified” criminal background result from the Department of Human Services and not be on the OIG exclusions list.
• Must be screened for TB.
• High school diploma or GED is preferred.
• Must adhere to Bethesda’s corporate compliance, policies, and procedures.
• Regularly required to use hands and fingers to examine residents, handle objects, tools, or controls.
• Requires walking, bending, stooping, pushing, pulling, kneeling, reaching overhead, and lifting up to 50 pounds.
• Must have adequate vision and hearing necessary to perform job functions.
