About the Role
Provide quality personal care and supportive tasks under the direction of an RN Case Manager or Team Leader, following an established Plan of Care.
Responsibilities
- Assist with personal hygiene including bathing, grooming, and oral care
- Provide peri-care and Foley catheter maintenance
- Assist with dressing patients
- Perform patient transfers and positioning while ensuring safety
- Monitor and record vital signs and weights
- Assist with ambulation and range of motion exercises
- Perform simple, non-sterile dressing changes as directed
- Assist with self-administered medications
- Communicate patient condition changes to the RN Case Manager
- Document care in compliance with organizational policies
- Maintain and clean medical equipment
- Provide nutritional support, feeding assistance, and light meal preparation
- Perform light housekeeping duties
Requirements
- Current, active Texas nurse aide certification (CNA)
- Listed in good standing on the Texas nurse aide/misconduct registry
- Valid Texas driver’s license
- Auto liability insurance
- Safe driving record
- CPR certification
