About the Role

The Care Manager is responsible for providing services to individuals in their assigned program, including crisis intervention, service plan development, guidance, support, and advocacy to help participants achieve their goals.

Responsibilities

  • Providing general information about the program and crisis intervention services.
  • Screening applicants for program acceptance.
  • Orienting participants to the program and conducting intake interviews to assess needs and strengths.
  • Developing comprehensive, goal-oriented service plans in conjunction with participants and their support network.
  • Providing guidance and support to assist participants with basic needs, service plan goals, challenges, and coping.
  • Facilitating referrals and advocating for participants to secure services, entitlements, and support.
  • Sharing information with program staff and collaborating with outside service providers for coordinated care.
  • Providing individual, group, and family counseling aligned with case plans.
  • Facilitating workshops, support groups, and activities to develop living skills and enrich participants' lives.
  • Monitoring participant compliance with program rules, intervening in crises, and applying consequences.
  • Monitoring progress toward service plan goals, establishing new goals and strategies as needed.
  • Participating in interviews, case reviews, staff meetings, training, and other activities.
  • Monitoring confidential participant files, case notes, records, and reports.
  • Assisting participants with discharge planning and follow-up contact.
  • Providing general office, operation, and program management support.
  • Participating in planning efforts to evaluate program effectiveness and enhance quality.
  • Obtaining ongoing training for professional growth and effective performance.

Requirements

  • BA/BS in a human service field or equivalent combination of education, training, and experience.
  • Two years of related work experience with the youth population.
  • Competent reading and writing skills.
  • Strong communication skills.
  • Excellent organizational skills and ability to prioritize job demands.
  • Sensitivity toward the program's target population.
  • Ability to relate comfortably and effectively to individuals of diverse backgrounds.
  • Respect for cultural differences.
  • Responsibility, non-judgmental attitude, and ability to work independently and as part of a team.
  • Strong commitment to personal and professional ethics and standards.
  • Ability to set and maintain appropriate boundaries with program participants.
  • Dependability and flexibility.
  • Valid NYS Driver's License.
  • Valid license to transport clients as necessary.
  • Ability to travel to various worksites.
  • Proof of valid car insurance policy when using a personal vehicle for travel.
  • Ability to climb multiple sets of stairs.
  • Ability to lift a minimum of 25 pounds.
  • Home or cellular telephone required.
  • Basic computer literacy necessary.
  • Specific clearances may be necessary depending on the position.
