About the Role

The Assistant Program Director will supervise case management services for adults with histories of homelessness and severe mental illness, ensuring quality care and compliance with regulations. This role involves assisting the Program Director with overall program administration and staff supervision.

Responsibilities

  • Ensure program compliance with Federal, State, and Local laws, regulations, and HRA/DSS requirements.
  • Meet housing and performance outcomes on a monthly, quarterly, and annual basis.
  • Assist the Program Director in program administration, overseeing service delivery, contract compliance, and targets.
  • Provide and ensure quality services through direct supervision of staff regarding case management.
  • Ensure timely and accurate completion of all documentation by direct service staff.
  • Develop and implement individual client service plans in conjunction with clinical staff, counseling team, and clients.
  • Ensure client contacts align with service plan goals and assist wellness coaches with intervention development.
  • Coordinate annual functional assessment reviews, psychosocial reviews, and chart audits for each client.
  • Maintain contact with other service agencies and ensure client placement for treatment and program activities.
  • Coordinate orientation and program entry for new residents and participate in discharge planning.
  • Serve as a liaison to community services, attend meetings, develop new linkages, and provide emergency coverage.
  • Prepare reports for funders, Regional Director, and AVP of Behavioral Health Housing Services.
  • Conduct and document supervisory meetings, promote staff development, and evaluate performance.
  • Ensure accurate, confidential, and prompt completion and submission of all client and program documentation.
  • Ensure thorough and timely incident reporting and follow-up.

Requirements

  • Understanding, appreciation, and commitment to the philosophy and mission of Services for the UnderServed (S:US).
  • Expertise in harm reduction and recovery-oriented services.
  • Master’s degree in social work or related field.
  • Three to five years of human services experience.
  • Two years of supervisory experience.
  • Familiarity with Federal, State, and Local laws and regulations as they apply to supportive housing programs and HRA/DSS regulation/requirements for NY 15/15 Supportive Housing.
