About the Role
Outward is seeking proactive and passionate Community Outreach Officers to join their team. This community-based role supports adults with learning disabilities to achieve independence by developing new skills, finding social activities, managing housing, finances, and maintaining a healthy lifestyle. The position offers a unique blend of direct support, service coordination, and professional networking.
Responsibilities
- Assist with claiming benefits, including form completion and DWP liaison.
- Provide support with budgeting and financial planning.
- Assist with job applications, including CV and form completion.
- Encourage and facilitate participation in meaningful activities.
- Identify and address safeguarding concerns.
- Support individuals with at-risk tenancies, including legal advice liaison and landlord communication.
- Promote healthy lifestyles and connect individuals with healthcare services.
- Attend appointments and provide detailed written feedback.
- Prompt and encourage household chores, cooking, and shopping to foster self-reliance.
- Assist in accessing community resources and local services.
- Provide prompting and encouragement for personal care.
- Provide prompting and encouragement for medication management.
- Engage in skill-sharing and supervision to promote self-reliance.
Requirements
- Good understanding of safeguarding and risk management in a care/support setting.
- Knowledge of health and safety issues in supported housing or care environments.
- Understanding of confidentiality, GDPR, and data protection principles.
- Access to a smartphone for SMS and authentication apps.
- Right to Work in the UK (visa sponsorship not provided).
