About the Role

Responsibilities

  • Provide support and service linkages to residents within the program.
  • Coordinate services and ensure quality care to residents.
  • Manage the operation of the Supported Housing program and assigned caseload.
  • Visit apartments routinely to assess resident functioning and needs.
  • Act as a liaison with community health, mental health, educational, vocational, and recreational programs.
  • Supervise and collect urine samples, ensuring accurate testing procedures.
  • Provide linkage services with detoxification and rehabilitation treatment centers.
  • Assist consumers in the housing selection process, including furnishings and utility setup.
  • Facilitate sublease and rental payments.
  • Facilitate housing subsidies, food stamps, and other benefits.
  • Assist consumers in linking to community support services.
  • Familiarize consumers with tenant rights and assist with landlord communication.
  • Develop individual support plans addressing self-care, money management, medication management, daily living skills, and symptom management.
  • Maintain all required written documentation, including consumer and program records.
  • Identify maintenance and safety needs of assigned apartments.
  • Report and document all incidents to supervisors within 24 hours.
  • Be available on-call 24/7 to respond to emergencies.
  • Provide weekly supervision of assigned staff, delegating tasks and offering direction.
  • Participate in meetings and trainings at various sites.
  • Lift/carry packages and boxes of supplies; clients if necessary.
  • Perform basic mathematical calculations for money management.
  • Communicate information verbally and in writing to staff and supervisors.
  • Participate in programs at various sites and times as needed.
  • Complete required training, including AMAP, CPR, and First Aid.
  • Participate in activities of other staff members during their absence or staff shortages.
  • Represent the agency at specified meetings and trainings.
  • Maintain confidentiality at all times.

Requirements

  • Bachelor’s degree in Social Services preferred.
  • Experience working with mental health consumers with/without co-occurring addictive disorders.
  • New York State driver’s license.
  • Access to a vehicle.
  • Ability to count, add, subtract, and manage money.
  • Ability to lift/carry packages and boxes of supplies; clients if necessary.
