About the Role
The Case Manager works with clients to assess their strengths and needs, plan a course of action, link them to services, provide counseling, and monitor progress. The role ensures clients receive appropriate, competent, and professional information, advice, referrals, and advocacy services for successful outcomes. This position may involve supervising direct care staff.
Responsibilities
- Manage the client intake and assessment process to orient the client to the program and produce a comprehensive assessment of client service needs.
- Develop with the client a mutually agreed-upon individual service plan reflecting assessment findings and goals, updating the plan as required.
- Meet regularly with the client to assist them in obtaining needed services/benefits to achieve their individual service plan goals.
- Provide supportive counseling to strengthen the client's ability to make appropriate life decisions.
- Document case management activities in accordance with agency and funder guidelines and procedures.
- Collect service data and participate in evaluation and quality assurance activities.
Requirements
- Demonstrated ability in verbal and written communication.
- Basic computer skills.

- High School Diploma or GED with 2 years of relevant shelter experience
