About the Role

The Family Physician is a core clinical provider at the London site, working collaboratively to offer comprehensive primary care across the lifespan for SOAHAC clients. The role supports longitudinal, relationship-based care and clinical decision-making while coordinating with internal and external partners.

Responsibilities

• Provide comprehensive primary care for SOAHAC clients, including health promotion, disease prevention, acute care, and chronic disease management.
• Conduct assessments, develop care plans, provide follow-up, and coordinate ongoing primary care for SOAHAC clients.
• Collaborate with Nurse Practitioners and other interdisciplinary providers within their respective scopes of practice.
• Participate in interdisciplinary case discussions, case conferences, and integrated care planning.
• Maintain accurate and timely clinical documentation in accordance with CPSO standards, organizational policies, and applicable privacy legislation (including PHIPA).
• Practice in a trauma-informed, culturally safe, and client-centered manner, recognizing the social determinants of health affecting Indigenous communities.

Requirements

• Medical Degree (MD) and Certification in Family Medicine (CCFP), or equivalent.
• Licence for Independent Practice, or eligibility, with the College of Physicians and Surgeons of Ontario (CPSO).
• Experience working with Indigenous communities, community health centres, or underserved populations is an asset.
• Demonstrated ability to work effectively within an interdisciplinary, team-based model of care.
• Commitment to ongoing learning in Indigenous cultural safety, trauma-informed care, and anti-oppressive practice.
