About the Role

Directs, supervises, evaluates, and provides physical therapy services to patients in the home, following physician’s orders.

Responsibilities

  • Evaluate patient using diagnostic muscle, nerve, joint, and functional ability tests and measures.
  • Prepare an assessment of the patient’s condition at the time of the initial evaluation and perform periodic re-evaluations.
  • Develop a written problem list and plan of care including types and frequencies of treatment.
  • Recommend modification of plan of care to physician, as needed.
  • Document clinical notes on the date of the visit into medical record and keep all records and summaries up-to-date.
  • Treat patients to address impairments and functional limitations, with a goal of reaching maximum functional independence.
  • Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic, and durable medical equipment.
  • Include and educate the patient and caregiver(s) regarding treatment plan and goals.
  • Attend physical therapy staff meetings and in-service presentations.
  • Participate in case conferences.
  • Participate in the development and periodic revision of the physician’s Plan of Treatment.
  • Assure appropriate safety practices and maintenance of equipment.
  • Perform other duties as assigned.

Requirements

  • Current, valid Physical Therapy license issued by NYS Department of Education.
  • One year of practical experience as a licensed physical therapist.
