About the Role
Change Grow Live is a charity dedicated to improving the lives of service users. This role focuses on providing specialist clinical interventions to individuals who are homeless or at risk of homelessness, helping them to change their lives and live to their full potential.
Responsibilities
- Carry out physical and psycho-social assessments for the homeless population using nursing skills and knowledge.
- Provide clinical/therapeutic treatments based on best evidence to improve health outcomes for the homeless population.
- Provide screening for blood borne viruses (BBV) through dry blood spot testing.
- Work within and consolidate integrated care and treatment pathways for individuals infected with BBV/s (Hepatitis B, Hepatitis C and/or HIV).
- Provide Hepatitis A and/or B vaccinations when indicated.
- Partner with other community teams and voluntary organizations to ensure seamless care delivery to the homeless population.
- Act as an advocate for people experiencing homelessness, supporting their choices and promoting independence and self-care.
- Contribute to CGL Naloxone Strategy by training staff and individuals in the use of naloxone.
- Offer wound care assessment and basic wound dressing to service users.
- Organize signposting to other services and ensure clear pathways into other treatment arenas.
Requirements
- First-level registered nurse with recent relevant post-qualification experience.
- Current nursing PIN.
- Sound and demonstrable knowledge of harm reduction and health promotion interventions.
- Ability to assess and recovery plan service users.
- Ability to formulate written reports.
- Clinical auditing skills to inform service evaluation and identify development areas.
- Professional, empathetic, and non-judgmental attitude towards service users and stakeholders.
- Understanding of professional integrity in relationships with service users, peers, and other professionals.
