About the Role
Responsible for assisting patients in reestablishing services by assessing their needs, coordinating documentation and telehealth appointments, managing pending cases, and ensuring effective communication with healthcare providers for comprehensive care.
Responsibilities
- Assist patients in reestablishing services by assessing their current skilled needs.
- Communicate with patients to understand their healthcare requirements and preferences.
- Obtain necessary paperwork from primary care physicians.
- Schedule telehealth appointments as needed to facilitate care.
- Manage pending cases within the GHC and EHC buckets.
- Make progress notes for patients post 30 days.
- Follow up on patients who have refused services.
- Address cases awaiting family decisions pending over 30 days.
- Collaborate with team members and healthcare providers to ensure timely follow up and comprehensive patient care.
- Maintain accurate and organized documentation of all patient interactions and statuses.
- Assist in covering absent employees and training new staff within their area of expertise.
Requirements
- At least two (2) years’ experience as a medical assistant, medical coordinator, or medical office receptionist.
- At least one year of previous healthcare experience.

Experience Level

- High school graduate or equivalent
- Two years of college preferred
Salary/Compensations
- $21 - $25 per hour
