About the Role
The Physical Therapist (PT) evaluates and assesses referred patients, formulating treatment plans within a home care setting. The PT delivers therapy services as defined in treatment plans, collaborating with physicians and case managers. This role allows for creativity and latitude in performing various tasks.
Responsibilities
- Evaluate/assess needs of referred patients.
- Formulate treatment plans in a home care environment.
- Provide therapy services defined in treatment plans.
- Work cooperatively with physicians and case managers.
- Perform a variety of tasks with creativity and latitude.
Requirements
- Graduate of an accredited Baccalaureate, Master's or Doctorate program in Physical Therapy.
- Current and unencumbered license to practice as a Physical Therapist in the state of employment.
- Two (2) years of appropriate experience as a Physical Therapist.
- Demonstrates good verbal and written communication and organization.
- Possess and maintain valid CPR certification obtained in-person with a practical skills evaluation.
- Access to reliable transportation.
