About the Role

The Housing Case Manager (HCM) provides comprehensive Case Management services and care coordination to residents living with HIV and/or co-occurring conditions. The HCM works closely with clients in their homes, in the office, and in the community to ensure housing stability by promoting independent living skills, financial management, and health and wellness.

Responsibilities

• Provides case management services to assigned clients, including supportive counseling, motivational interviewing, crisis intervention, care coordination, linkage and referral, assessment and service plan development, field work, and assessment/observation of resident in their living space as needed but no less than once per month.
• Documents all services and interventions provided as per funder, regulatory, industry and internal standards.

Requirements

• A minimum of three years of relevant experience in a social services setting working with individuals who have experienced homelessness, mental health issues, and/or substance use is required.

Salary/Compensations

• $24/hr - $31.25/hr commensurate with experience
