About the Role

The Housing Case Manager (HCM) provides comprehensive Case Management services and care coordination to residents living with HIV and/or co-occurring conditions. The HCM works closely with clients in their homes, in the office, and in the community to ensure housing stability by promoting independent living skills, financial management, and health and wellness.

Responsibilities

  • Provides case management services to assigned clients, including supportive counseling, motivational interviewing, crisis intervention, care coordination, linkage and referral, assessment and service plan development, fieldwork, and assessment/observation of resident in their living space at least once per month.
  • Documents all services and interventions provided as per funder, regulatory, industry, and internal standards.
  • Provides referrals and follow-up for primary care, substance use services, nutrition, clothing, mental health services, and financial management.
  • Maintains meaningful engagement with residents using motivational interviewing and other supportive interventions to help residents achieve and maintain the highest level of independent functioning.
  • Promotes participation in community activities and other endeavors that foster individual growth and development of a social support network.
  • Utilizes internal services to connect and coordinate with other departments including ADHC, Health Services, OASAS, Health Homes, Behavioral Health, Community Partnerships, Legal, and Job-Training.
  • Assists new clients with moving into apartments and transitioning from homelessness to stable housing.
  • Develops strength-based individualized goals in collaboration with the client and provides constructive feedback on goal participation.
  • Accompanies residents to facilitate access to external services as needed.
  • Assesses and discusses financial management with the client, including rent obligations, subsidy maintenance, utility payments, and food/nutritional needs.
  • Completes apartment inspections and places work orders, collaborating with custodial and operations staff.
  • Actively participates in weekly supervision meetings, coming prepared.
  • Completes all required reports and paperwork by prescribed deadlines.
  • Demonstrates excellent customer service with clients, staff, vendors, and stakeholders.
  • Coordinates care, including case conferencing, with residents’ internal and external service providers.
  • Participates in case conferences, internally or externally, as part of a multidisciplinary team.
  • Performs other responsibilities and duties as assigned.
  • Enthusiastically participates in advocacy and direct actions to end homelessness and HIV/AIDS.

Requirements

  • Bachelor's degree in social work, psychology, counseling, or a related field preferred; Master's Degree a plus.
  • A minimum of three years of relevant experience in a social services setting working with individuals who have experienced homelessness, mental health issues, and/or substance use is required.
  • Ability to prioritize and multi-task.
  • Experience with and proven commitment to working with low-income and marginalized communities, as well as individuals engaging in substance use, and having multiple chronic medical and/or behavioral health conditions.
  • Ability to document in various electronic systems to chronicle client services.
  • Dependable and flexible.
  • Able to maintain a high level of client confidentiality.
  • Excellent internal and external customer service skills and strong interpersonal skills.
  • Good communication skills, both verbally and in writing, including the ability to communicate with all levels of management.
  • Proficient in Microsoft Outlook, Word, and Excel.
  • Ability to work with colleagues to solve problems.
  • Familiarity with Harm Reduction practices.
  • Must be present in the program site 100% of the time.
  • Working knowledge of NYC mass transportation systems (subway and bus) is required.
  • Occasionally expected to de-escalate tense, even hostile, client situations.

  • $24/hr - $31.25/hr commensurate with experience
