About the Role
VHA Home HealthCare is seeking a Registered Practical Nurse or Registered Nurse to support our Palliative care program in the Central region. This visiting nurse role focuses on providing and coordinating home health nursing services for individuals, families, and groups, ensuring high-quality, client-centred care in accordance with College of Nurses of Ontario standards.
Responsibilities
- Assess, monitor, and coordinate care planning.
- Evaluate care plan interventions through ongoing reassessment.
- Ensure quality patient care by identifying factors affecting practice autonomy.
- Demonstrate professionalism, leadership, and accountability in client interactions.
- Support safe client care through continuous quality improvement.
- Inform nursing supervisors of client concerns or changes in condition.
- Maintain documentation standards according to the College of Nurses of Ontario.
- Participate in a standby/on-call rotation.
- Participate in team meetings as required.
Requirements
- Registered and in good standing with the College of Nurses of Ontario (CNO).
- RPN diploma or RN diploma/degree from a recognized university or college.
- Valid First Aid or CPR Certification.
- Valid Ontario driver's license (G2 or G).
- Access to a reliable vehicle for traveling.
- Effective written and verbal communication skills.
- Reliable attendance, punctuality, and availability.
