About the Role
Provide critical crisis intervention, protective services, and social casework for older adults who are mentally or physically impaired, abused, or neglected. This role operates on both a voluntary and involuntary basis, requiring professional judgment and a commitment to elder justice.
Responsibilities
- Conduct preliminary screenings and triage for emergency interventions; provide immediate information and referral services to clients and families.
- Perform in-home and in-office assessments of social and emotional needs using standardized instruments.
- Develop written case plans including individual/family counseling, benefit screening, and arrangement of services.
- Evaluate clients' capacity to manage their affairs; assist in securing financial management or guardianship to prevent elder abuse and financial exploitation.
- Conduct ongoing in-home visits to monitor risk, evaluate functioning, and revise case plans in real-time.
- Participate in a rotating on-call schedule (evenings/weekends/holidays). Respond to crisis situations requiring emergency medical or court-ordered actions.
- Complete required case records, reports, and statistics within mandated timeframes utilizing specialized software.
Requirements
- Graduate of an accredited college or university with a BSW or BA/BS (Psychology or Gerontology preferred).
- Computer literacy and the ability to maintain meticulous records.
- Proficiency in English required; knowledge of Yiddish, Russian, Spanish, or Patois is helpful.
- Ability to travel to clients’ homes using mass transit and navigate physical barriers (stairs, etc.) in residences.
- Ability to understand and appreciate the roles of race, religion, ethnicity, and sexual orientation in serving a diverse client base.
