About the Role

Assist each child with obtaining permanency, maintaining safety, and ensuring a healthy well-being while in care. Coordinate and monitor client services and act as a liaison to ACS and community resources for discharge and reunification.

Responsibilities

  • Create service plans addressing safety, behavioral, educational, psychological, and permanency needs in partnership with the child, biological family, and foster family.
  • Monitor safety, well-being, and progress against critical outcomes through regular casework contacts and documentation.
  • Schedule and supervise mandated visits between child and natural family.
  • Conduct required interviews with child, natural family, and foster family.
  • Make independent field visits to homes, schools, courts, prisons, hospitals, and other sites as needed.
  • Attend weekly supervision to discuss case situations and follow supervisor directions.
  • Attend Family Court hearings and provide testimony.
  • Follow up on court directives.
  • Participate in Family Team Conferences and other meetings to advance permanency.
  • Collaborate with Integrated Services (Medical, Mental Health, Case Management) to monitor client needs and ensure appropriate services.
  • Utilize data to measure performance and ensure all outcomes are met.

Requirements

  • Bachelor's degree required.
  • Knowledge of Child Welfare System preferred.
  • Excellent written, oral, organizational, and interpersonal communication skills.
  • Collaborative team player.
  • Proficiency in Microsoft Office, including Outlook and Excel.
  • Computer proficient.
  • Knowledge of Connections.
  • Ability to work flexible hours, including some evenings.
  • Ability to use public transportation or Agency vehicle for travel throughout New York City.
  • Bilingual preferred.
