About the Role
As a Client Support Worker, you will play a vital role in promoting the well-being and independence of program participants within the Street to Home Supportive Housing program. You will work in partnership with clients to support their goals, enhance their quality of life, and foster an inclusive, supportive environment through goal setting, care planning, and life skills training.
Responsibilities
- Build positive, respectful relationships with program participants and provide emotional support.
- Deliver one-on-one support aligned with Coordinated Care Plans, including form completion and resource navigation.
- Participate in the intake process, including screening, interviews, onboarding, and follow-up.
- Monitor client progress and well-being, contributing to care plan evaluations.
- Support goal setting and care planning to promote independence and community integration.
- Assist with activities of daily living using ADL/IADL assessment tools.
- Facilitate housing stabilization by assisting with applications and liaising with housing organizations.
- Encourage participation in community activities and identify services to meet individual needs.
- Collaborate with external service providers and community partners.
- Respond to crises and distress behaviors using de-escalation and trauma-informed approaches.
- Perform delegated health-related tasks, including medication management.
- Arrange and accompany clients to appointments and community activities.
- Maintain accurate documentation and case notes.
- Uphold health and safety standards, including emergency procedures and fire regulations.
- Contribute to program development and evaluation initiatives.
Requirements
- Social Service Worker diploma or related education.
- 2+ years of experience supporting older adults with lived experience of homelessness, mental health concerns, or substance use.
- Ability to lift up to 40 lbs.
- Availability for evening and weekend shifts as required.

- Written, oral, and interpersonal communication skills.
- Conflict resolution and crisis intervention.
- Computer literacy with databases and web-based applications.
- Knowledge of geriatric and aging issues.
- Knowledge of harm reduction approaches.
- Knowledge of the Mental Health Act.
- Knowledge of community resources and systems in Toronto.
