About the Role

This role delivers essential services that foster self-sufficiency and housing stability within WoodGreen’s Street to Home Housing Program. The Case Manager provides specialized support to tenants navigating mental health and addiction challenges or chronic homelessness, with a focus on hoarding and pest control management. This position is vital to maintaining affordable and sustainable housing.

Responsibilities

  • Provide comprehensive case management services including assessment, personal goal setting, case planning, informal counselling, follow-up support, crisis intervention, service co-ordination, case conferencing, and referrals.
  • Undertake needs assessments and develop case plans with individuals in the program.
  • Assist tenants with mental health issues by providing day-to-day physical, emotional, and social supports, life skills assistance, information, and resources.
  • Identify and address mental health issues, connecting clients with primary care and other needed resources.
  • Support clients with substance use and mental health issues to maintain their housing and advocate on their behalf.
  • Accompany clients to offsite medical appointments and other community-based services, providing emotional and logistical support.
  • Link clients to various community resources and facilitate appropriate referrals.
  • Ensure the coordination of services and a continuum of care.
  • Liaise, consult, and collaborate with other program staff and service providers.
  • Monitor housing challenges and resident adherence to policies, developing intervention strategies.
  • Complete statistics/reports and maintain program/activity enrollment records.
  • Ensure anti-racism objectives and community development principles are achieved.
  • Gather statistical and narrative information for service and organizational reporting.
  • May be exposed to second-hand smoke, behavioral & cognitive outbursts, death, dying, drug use, and pest-related concerns.

Requirements

  • 3+ years related experience in mental health and harm reduction, housing help, and working with those who have experienced homelessness.
  • Experience working effectively in a team in collaboration with other community professionals.
  • Proven ability in case management, informal counselling, and crisis intervention.
  • Able to work independently with minimal supervision.
  • Ability to exercise good judgment, flexibility, creativity, and sensitivity to changing needs.
  • Proven ability to minimize barriers through service delivery for low-income individuals with mental health and substance use issues.
  • Demonstrated ability to develop, implement, and evaluate community needs and follow-up programs.
  • Demonstrated interpersonal skills.
  • Excellent verbal and written communications skills.
  • Demonstrated information and referral skills; and ability to effectively link clients to supportive services.
  • Ability to work from a client-centered approach.
  • Comprehensive understanding and knowledge of the health sector and its related services.
  • Excellent knowledge of community resources.
  • Demonstrated ability to follow policies, procedures, and adhere to legislation.
  • A thorough understanding of clinical practices and care models.
  • Ability to exercise good judgment, flexibility, and contribute to the effective functioning of clients.
  • Proven ability to facilitate presentations and workshops.
  • Applied computer skills (MS Word, Excel, and Internet).
  • Ability to work weekends, statutory holidays on an exceptional basis, where and if applicable.
  • May work in environments where there are pests.
  • May work in environments where clients use substances.
  • May work in environments wherein clients have incontinence issues.
  • Clients may exhibit responsive behaviours including profanities and expletives.
  • Able to lift 40lbs.

  • External Rate: G7 - $56,802.64
