About the Role
The Homeward Bound program assists female-led families in unstable conditions to transition to sustainable employment, permanent housing, and independent living. This role involves providing case management, counseling, and support services to clients.
Responsibilities
- Perform case work duties including assessment, goal setting, action plan development, ongoing intervention, re-assessment, case termination, and follow-up using professional case management practices and clinical modalities.
- Coordinate client participation in all aspects of the Homeward Bound program.
- Liaise with Community Colleges and academic upgrading providers to ensure participant enrollment and full participation in programs and on-the-job training.
- Ensure clients prioritize meeting program goals and milestones.
- Liaise and coordinate with Toronto Children Services and Toronto Employment and Social Services.
- Assist women in resolving functional problems of daily living, provide advocacy, and facilitate access to support services.
- Identify barriers to learning and refer participants to appropriate support sources.
- Respond to emergencies and provide crisis intervention.
- Maintain a caseload.
- Maintain client records in a centralized database according to program policy.
- Collect statistical data according to central and unit policies.
Requirements
- Master of Social Work or Master’s Degree in a related field. Candidates with BSW and related experience will also be considered.
- Registration with the Ontario College of Social Workers and Social Service Workers or its equivalent is preferred.
- Minimum of two years of related work experience with women who have experienced homelessness, violence, or related issues at a case management level.
- Experience in employment planning and preparation work.
Skills
- Extensive experience in a variety of counselling modalities, especially related to working with women who have experienced homelessness or domestic violence.
- Demonstrated case management skills.
- Demonstrated group facilitation skills.
- Strong knowledge of career planning and job search techniques.
- Provide on-going feedback to enhance employability of all clients.
- Demonstrated intervention skills, including crisis intervention.
- Demonstrate on-going self-reflection to ensure best practices in service delivery.
- Social assessment skills.
- Knowledge and skills in conflict resolution.
- Well-developed and timely documentation skills.
- Strong computer skills, including internet research.
- Knowledge of community development.
- Skills in the use and referral to social resources.
- Collaboration and teamwork skills.
- Able to work under pressure.
- Demonstrated experience working with clients from diverse communities.
- Understanding of anti-oppressive practices.
- Understanding of trauma-informed approach.
- Lived experience as a member of the BIPOC community, or other experience of social identity intersectionality.
