About the Role
The Supportive Housing Program at WoodGreen provides stable housing and comprehensive supports for seniors who have experienced street involvement and may face mental health or addictions challenges. This position plays an essential role in supporting individual clients and fostering community engagement, contributing to an inclusive, supportive, and enriching living experience for residents.
Responsibilities
- Develop, organize, facilitate, and evaluate social, recreational, and educational programs for senior tenants.
- Assess client needs to develop new initiatives.
- Supervise students and volunteers to strengthen program delivery.
- Provide administrative support to ensure high-quality service.
- Assist the Manager in developing policies and procedures.
- Plan, build, organize & facilitate onsite activities with staff and external partners.
- Engage with tenants to assess programming interests.
- Assess, develop, and build healthy and engaging program spaces.
- Facilitate skill-building, healthy living and fun workshops.
- Create a calendar of planned monthly programming.
- Liaise with external parties and support staff with workshop and activity development/co-facilitation.
- Create a participant intake pathway & marketing materials.
- Create & implement monthly/quarterly Pre & Post-program surveys.
- Interact with individuals in person, providing emotional support, and feedback to case managers/counsellors.
- Encourage participation in programs.
- Work with the case manager to carry out care plan goals and expectations.
- Complete daily needs assessments to determine appropriate programming.
- Monitor clients, identify additional needs and mitigate risks.
- Identify participants interested in becoming peer helpers.
- Collaborate with the community and build partnerships for new programming.
- Work collaboratively with community partners to maximize client support and create pathways to recovery goals.
- Ensure anti-oppression, community development, and harm reduction principles are achieved.
- Carry out additional duties as identified by Management.
- Take part in team and unit meetings.
- Participate in ongoing training and education.
- Participate in regular supervisory sessions and annual performance appraisals.
- Help to create a healthy and motivating work environment.
- Document client interactions on provided software.
- Collect, input and extract statistical information using provided software.
- Be prepared to work in environments with pest control, hoarding, death and dying, and drug use.
- Be prepared to work in environments where clients may use substances, have incontinence issues, or exhibit responsive behaviours.
Requirements
- Bachelor-level degree or Social Service Worker with 5+ years’ experience creating and facilitating programming.
- 5+ years of related experience working with adults/seniors, homeless individuals with mental health/substance use issues.
- Demonstrated experience supporting adults/seniors with mental health, substance use, housing needs, etc.
- Proven ability to lead, develop and facilitate programming.
- Strong assessment skills.
- Demonstrated ability to develop, implement and evaluate client/tenant needs.
- Demonstrated interpersonal skills.
- Excellent verbal and written communications skills.
- Demonstrated information and referral skills.
- Ability to effectively link clients to supportive services.
- Self-directed and independent, but also experienced in working effectively as a member of a team.
- Proven ability to minimize barriers through appropriately designed and managed service delivery.
- Ability to work from a client-centered approach.
- Comprehensive understanding and knowledge of the health sector and its related services.
- Excellent knowledge of community resources.
- Demonstrated ability to follow policies, procedures, and adhere to legislation.
- Well-grounded; good boundary setting skills.
- Travel may be required while on shift for community building/partnership meetings.
- A thorough understanding of clinical practices and care models.
- Ability to exercise good judgment, flexibility, and contribute to the effective functioning of clients.
- Strong leadership skills.
- Proven ability to facilitate presentations and workshops.
- Applied computer skills (MS word, excel, and Internet).
- Ability to use case management software is an asset (Pirouette).
- Ability to work weekends & statutory holidays on an exceptional basis.
- Able to lift 40lbs.
