About the Role

The Behavioral Health Clinician-Collaborative Care (BH-CC) is a core member of the collaborative care team, supporting and coordinating the mental and physical health care of patients. This role involves working closely with medical providers, psychiatric consultants, and the primary care team to ensure comprehensive patient care.

Responsibilities

• Support the mental and physical health care of patients on an assigned patient caseload.
• Coordinate care with the patient’s medical provider and other mental health providers.
• Screen and assess patients for common mental health and substance abuse disorders.
• Facilitate patient engagement and follow-up care.
• Participate in warm hand-offs from primary care and the SBIRT process.
• Coordinate care with behavioral health services (mental health and substance use disorder).
• Provide patient education about common mental health and substance abuse disorders and available treatment options.
• Track treatment response and monitor patients for changes in clinical symptoms and treatment side effects.
• Support psychotropic medication management, focusing on adherence, side effects, and effectiveness.
• Provide brief behavioral interventions using evidence-based techniques via telephonic/tele-practice contact.
• Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments.
• Participate in caseload consultations with the psychiatric consultant and communicate treatment recommendations.
• Participate in primary care meetings/huddles and behavioral health staff meetings.
• Participate in behavioral health clinical supervisory and education meetings.
• Utilize the electronic medical record effectively.
• Track and document encounters, patient progress, and treatment recommendations in the EHR.
• Facilitate referrals for clinically indicated services outside of the organization.
• Develop and complete relapse prevention self-management plans with patients.
• Contribute to a safe and respectful treatment environment.
• Develop and network with community service providers for a full continuum of care.
• Maintain professional relationships with staff, providers, and the community.
• Participate in individual and group supervision and report pertinent information to supervisor.
• Utilize the DSM V Diagnostic and Statistical Manual in diagnostic impressions.
• Complete Continuing Education requirements and maintain professional licensure.
• Demonstrate excellence in customer service.
• Communicate HIPAA compliance, corporate compliance, and client confidentiality.
• Ensure compliance with local, state, and federal regulations and accreditation standards.
• Complete other duties as assigned.

Requirements

• Bachelor’s degree in a behavioral health related field (social work, psychology, or human services), or be a clinician (limited permit) with behavioral health training.
• Formal education or specialized training in behavioral health such as social work, counseling, psychology, or human services.
• Master’s Degree in Social Work or Master’s degree in Mental Health Counseling with an LMSW from New York State or Master’s Degree in Mental Health Counseling with an LMHC.
• At least one year of work experience providing behavioral health treatment services in a medical setting.
• Experience with assessment and treatment planning for common mental health and/or substance use disorders.
• Strong assessment and diagnostic skills.
• Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions.
• Basic knowledge of psychopharmacology for common mental health disorders within the appropriate scope of practice.
• Knowledge of recovery concepts, self-help groups, and community resources.
• Demonstrated ability to collaborate and communicate effectively in a team setting.
• Provides excellent customer service and demonstrates a high level of cultural competency.
• Ability to adhere to strict confidentiality guidelines.
• Working knowledge of computers and electronic health records.
• Three to five years’ experience providing behavioral health treatment services in a medical setting.
• Experience with evidence-based counseling techniques.
