About the Role
Provide community In-Home services and assistance to seniors, empowering individuals and families through dedicated community support.
Responsibilities
- Ensure accurate inputting of case notes and older adult information in STARS daily.
- Ensure senior safety in their homes as required by FDNY.
- Perform In-Home services to eligible members and homebound seniors.
- Assist older adults with setting up Zoom links and encourage participation in virtual activities.
- Maintain and comply with standards set by DFTA and Department of Health & Mental Hygiene.
- Report potential unsafe conditions to the In-Home Coordinator immediately.
- Keep accurate and up-to-date records in a neat and organized manner.
- Maintain up-to-date resource files, materials, and contact information.
- Maintain accurate display of benefits and resources on bulletin boards.
- Conduct outreach of In-Home services at the center.
- Meet all deadlines for assigned duties, including In-Home Activity Calendar, Monthly Units of Service, and First Aid, AED, CPR.
- Maintain and meet all unit targets and goals.
- Keep all records, including enrollment reports and monetary contributions.
- Plan and coordinate recreational, social, educational, and health activities for In-Home services.
- Maintain an accurate list of support linkage groups.
- Refer seniors to social services support linkages by providing case assistance, information, and referral.
- Identify and resolve problems in a timely manner.
- Directly supervise all volunteers/consultants in the In-Home services department.
Requirements
- Must receive/obtain First Aid, CPR, AED training.
- Must have a valid Driver’s License with a clean Abstract.
- HS Diploma or equivalent.
- Experience working with the Older Adult population.
- Ability to perform each essential duty satisfactorily.
