About the Role
The Licensed Social Worker (LMSW)/Licensed Mental Health Counselor (LMHC) will provide oversight and direct services to individuals (including youth), and family therapeutic services to participants, including assessments, treatment plans, and discharge planning, under the direction of the Site Director of the Ulster County Crisis Support Center in partnership with community providers.
Responsibilities
- Address referrals from community providers, including referrals via warm hand-offs
- Engage in crisis de-escalation and therapeutic interventions to support individuals experiencing a crisis
- Lead daily huddles to assess needs and coordinate services
- Work in collaboration with individuals, families, community providers, team members, and external partners
- Complete and sign off on required documentation and paperwork including biopsychosocial assessments and treatment planning and implementation in a timely manner
- Demonstrate cultural competency and commitment to participants
- Maintain a Trauma-Informed environment and person-centered approach to care
- Demonstrate commitment to the philosophy of the agency
- Act as an advocate and help individuals with linkages to resources to address unmet needs
- Make psychosocial assessments and develop and implement a viable care plan
- Complete discharge planning and collaboration to ensure continuity of care
- Perform other duties assigned
