About the Role
The Peer Care Manager provides CTI services from a lived-experience and non-clinical perspective, drawing on personal experience with mental illness, substance use, homelessness, or trauma. This role involves developing person-centered care plans, sharing knowledge of community resources, providing linkages, engaging individuals in skill-building, offering crisis management, collaborating with providers, and assisting individuals in navigating complex systems and informal supports.
Responsibilities
- Complete all CTI, agency and state required trainings.
- Foster a beneficial working relationship with the partnering hospital by attending regular meetings and communicating effectively.
- Provide care coordination services for a maximum of 15 individuals.
- Link individuals to supports in the community to promote community tenure and management of their behavioral and physical health needs.
- Maintain documentation in the electronic health record according to CTI manual and agency policies.
- Provide CTI core services including person-centered planning, psychosocial rehabilitative services, crisis prevention, health services, and care coordination.
- Participate in weekly case presentations to review caseload and provide updates.
- Provide reporting data to the Team Lead.
- Adhere to the guiding principles of the trauma-informed approach.
- Attend case conferences to troubleshoot and coordinate care.
- Assist the Team Lead in maintaining an up-to-date community resources list.
- Participate in CTI Learning Collaborative and CTI Learning Community.
- Function as a member of a multi-disciplinary team.
- Must be part of the after-hours on-call rotation.
- Maintain client confidentiality at all times.
- Performs other duties as assigned or that may develop.
Requirements
- Must be at least 18 years of age.
- Must have attained a high-school diploma or equivalent.
- Must have at least six (6) months of direct care experience with individuals with serious mental illness.
- Must maintain valid driver’s license and acceptable driving record.
