About the Role
Impact Health PC is seeking a Nurse Practitioner or Physician Assistant in Poughkeepsie, NY, to deliver person-centered care in skilled nursing facilities. Enjoy a flexible Monday-Friday schedule with no nights, weekends, or holidays, and make a meaningful impact in a collaborative environment.
Responsibilities
- Deliver person-centered care using evidence-based practices
- Collaborate with interdisciplinary facility teams
- Conduct assessments, develop treatment plans, and manage follow-up care
- Educate patients, families, and staff
- Complete timely documentation (within 24 hours)
- Ensure compliance with state regulations in SNF settings
Requirements
- Active license as a Nurse Practitioner (NP) or Physician Assistant (PA)
- Minimum 2 years of post-certification experience
- Experience in SNFs, hospitalist roles, emergency departments, or ICU/SICU preferred
- Proficient with EHRs and clinical technology
- Strong communicator with a collaborative mindset
